By becoming a patient of our clinic, you acknowledge and agree to the following terms and conditions. These are designed to promote a respectful, safe, and effective therapeutic relationship between you and our clinic.


Medical Services

  • Our clinic provides comprehensive neurologic and epilepsy-related care, including consultations, follow-ups, diagnostic evaluations, and treatment planning. All care will be provided in accordance with current medical standards and clinical judgment.
  • Treatment decisions are made in partnership with you, but the physician reserves the right to determine the appropriateness and scope of care.

Appointments and Cancellations

  • Appointments must be scheduled in advance.
  • Please arrive on time. Late arrivals may result in rescheduling or shortened visits.
  • Cancellations must be made at least 24 business hours in advance to avoid a fee, as detailed in our Cancellation Policy.

Fees and billing

  • Patients are responsible for all charges.
  • Payment is due at the time of service unless prior arrangements are made.
  • Any disputes regarding billing must be submitted in writing within 30 days of the billing statement date.

Refunds

  • Medical services rendered are non-refundable.
  • Overpayments or billing errors will be reviewed and refunded or credited as appropriate, per our Refund Policy.

Communication

  • Secure messaging via email is preferred for non-urgent communication.
  • Voicemail messages will be returned within 1–2 business days.
  • Clinical advice will not be provided via unsecured channels.
  • In case of emergency, dial 911 or go to the nearest emergency room.

Medications and Refills

  • Prescription refills require an evaluation to ensure safety and appropriateness.
  • Please request refills at least 72 business hours in advance.
  • Controlled substances may require additional monitoring and agreement to a medication contract.

Privacy and HIPAA

  • We adhere strictly to federal HIPAA regulations to protect your personal health information.
  • Your records will only be released with your written consent or as required by law.

Patient Conduct

  • Patients are expected to treat providers and staff with respect.
  • Disruptive, threatening, or abusive behavior may result in dismissal from the practice.

Termination of Care

  • You may terminate your care at any time by notifying us in writing.
  • We reserve the right to discharge a patient for repeated missed appointments, non-compliance, or inappropriate conduct, with written notice and referral options as applicable.
